La tendinitis rotuliana es una lesión de la rodilla bastante común en ciclistas, runners, jugadores de voleibol, baloncesto, fútbol y atletas profesionales. Por esta razón, a menudo también se la conoce como “rodilla del saltador”, aunque en realidad puede afectar a cualquier persona cuya actividad habitual conlleva un gran esfuerzo para las articulaciones de las rodillas, ya sea por prácticas deportivas que incluyen saltos frecuentes, o por motivos laborales o por su estilo de vida.
De forma específica, esta dolencia consiste en la lesión del tendón rotuliano cuya misión es enlazar la tibia con la rótula. Es el tendón del extremo distal del cuádriceps y juega un papel clave a la hora de extender la rodilla y realizar acciones tan cotidianas como caminar, correr, subir/bajar escaleras, saltar, sentarse o agacharse.
Los síntomas más recurrentes de la tendinitis rotuliana suelen ser un dolor punzante y localizado en la zona por debajo de la rótula que se hacen patentes al flexionar o solicitar fuerza con la rodilla, o incluso en reposo. A veces puede dar sensación de inflamación en la zona inferior de la rótula por el engrosamiento que puede acabar desarrollando el tendón.
El dolor en la rodilla asociado a la tendinitis rotuliana va evolucionando, desde la fase inicial cuando se presenta durante el inicio de la actividad física para dar lugar a un dolor más agudo que aparece en reposo o durante el desarrollo de cualquier actividad de la vida diaria. Por esta razón es necesario realizar siempre un tratamiento profesional y específico. De lo contrario, se corre el riesgo de que se vuelva crónica, existiendo una mala reparación de los tejidos llegando a un estado de degeneración fibrilar o tendinosis
Existen diferentes causas que pueden desencadenar la tendinitis rotuliana. De hecho, factores congénitos y enfermedades de carácter metabólico como la diabetes, anomalías como la desviación de la rótula, desequilibrios musculares o rigidez en los músculos de las piernas pueden ser decisivos para desarrollar esta patología.
Sin embargo, de forma general, las causas más habituales de la tendinitis rotuliana suelen estar relacionadas con los pequeños microtraumatismos que se producen de forma frecuente durante ejercicios como correr y saltar sobre superficies duras o pedalear en exceso. Estos ejercicios realizados de manera intensa o sin la progresión y adaptación adecuada a la actividad física fuerzan el tendón rotuliano, obligándolo a sufrir una gran tensión que provoca microdesgarros y conllevan la aparición de inflamaciones y dolores.
Entre los factores que originan la tendinitis rotuliana y otras tendinopatías también es muy común la falta de estiramientos y calentamiento dinámicos previos a la práctica del deporte o el desarrollo de malas posturas y técnicas. Igualmente, los cambios de intensidad debidos a preparación de competiciones o la inadecuada planificación de entrenamientos, en definitiva una mala preparación física también suelen afectar a la irrupción de estas patologías.
Hay una gran variedad de tratamientos para la tendinitis rotuliana en función de cada persona y la gravedad de la lesión, que fluye en cinco niveles, siendo el 1 una tendinitis leve y el 5 una tendinopatía con degeneración del patrón estructural o tendinosis.
En cualquier caso, el trabajo previo para realizar un buen tratamiento deberá combinar la adecuación de la práctica deportiva, para dar paso al desarrollo de diferentes ejercicios específicos para la tendinitis rotuliana: ejercicios de fortalecimiento y estiramiento con los que tonificar los músculos de la rodilla de forma suave, siempre bajo la supervisión del profesional sanitario.
Para diagnosticar de forma específica sobre tu tipo de tendinopatía, tu especialista usará resonancias magnéticas, ecografías con las que desvelar el estado de tu tendón.
De forma usual, los casos de tendinitis leves suelen ser tratados mediante la aplicación de frío y hielo en la zona afectada, combinada con el vendaje y ciertos ejercicios. Para casos de gravedad media se puede optar por diferentes soluciones como la electroterapia, ejercicios de carga dinámica, kinesiotape, calor profundo. También son muy eficaces los tratamientos de fisioterapia invasiva, de los que te hablamos más adelante.
Otra de las opciones recurrentes que suelen recetar los médicos especialistas es el uso de medicamentos de tipo antiinflamatorio. Estos fármacos pueden ser indicados para mitigar el dolor en determinados casos. Así mismo, algunos terapeutas optan por aplicar corticoides sobre la piel mediante descargas eléctricas como metodología de tratamiento. Raras veces se opta por la cirugía.
Debes saber que no hay un tiempo de recuperación concreto para la tendinitis o tendinopatía rotuliana, cada caso es único por su grado de gravedad, así que la rehabilitación puede fluctuar entre un par de semanas y varios meses. Si no se toman las medidas adecuadas se puede cronificar incluso años.
La Electrólisis Percutánea Intratisular (EPI) puede ayudar de forma definitiva a acortar el tiempo de rehabilitación. Es un tratamiento de fisioterapia invasiva que a través de la aplicación de una corriente galvánica de baja intensidad sobre los tejidos lesionados, estimula la regeneración del tejido de forma natural y fisiológica.
Combinando la EPI y la Neuromodulación Percutánea Ecoguiada, que trabaja sobre el sistema nervioso y ayuda a mitigar el dolor, se obtienen muy buenos resultados en los casos de tendinitis rotuliana, especialmente porque se logra una recuperación eficaz, evitando recaídas.
Es imprescindible realizar dichos tratamientos de forma ecoguiada, ya que la visión ecográfica nos permite localizar el punto exacto de lesión y la precisión adecuada para el éxito y seguridad del tratamiento
Estos tratamientos se suelen aplicar en un ciclo de 4 sesiones durante el cual no se precisa de un reposo total o de parada de la actividad física, dependiendo del grado de lesión. Las sesiones se suelen combinar con ejercicios dinámicos específicos.
